Rules of the online shop - Centrum Medyczne ODENT Sp. z o.o.
- These terms and conditions define the conditions for making purchases in the odent.co.uk online shop.
- The Seller is Centrum Medyczne ODENT Sp. z o.o. conducting its activity in Warsaw at ul. Grzybowska 85/U4, 00-844 Warsaw, entered into the register of entities conducting medical activity conducted by the Mazovian Voivod under the following number: 000000253578, holding tax identification number NIP 525-288-78-15, statistical number REGON 0000937361.
- Orders can be placed in the online shop 24 hours a day and registration in this shop is completely free of charge and voluntary. In the odent.pl online shop, orders may only be placed by persons of legal age who have full legal capacity to enter into contracts for the purchase of goods and services.
- The information provided on the shop's website does not constitute an offer within the meaning of the Civil Code. Placing an order shall be treated as an offer to purchase the selected goods under the conditions presented in the product description.
- The placement of an order is confirmed by the Seller by e-mail. The order is realised after the payment for the goods specified in the order has been credited.
- Goods are dispatched to the Buyer's address specified in the order within 2-3 working days from the date of payment being credited to the Seller's account.
- The products offered in the online shop are legally introduced into the Polish market, brand new and free from legal and physical defects.
- The buyer has the right to withdraw from the order without giving reasons within 14 days of receiving the ordered goods. Notification of your wish to return the ordered product should be made to the e-mail address recepcja@odent.pl.
- If the Buyer discovers that the product does not conform to expectations, the Buyer is obliged to send a letter to the Seller's address, specifying the non-conformity and including expectations as to the fulfilment of the order.
- Complaints will be dealt with immediately, up to a maximum of 14 days.
- The right to 1TP2Complaint is:
- User on the Functionality of the Website;
- Buyer for the Products purchased by him.
- Complaints must be made in writing and sent by registered post to the registered address of the Owner given in paragraph 2 of these Terms and Conditions or by sending an e-mail to: recepcja@odent.pl
- A complaint should include at least: first name, surname, social security number, address, electronic mail address (e-mail) of the Buyer, Facility where the Service was provided (in the case of a complaint submitted by the Buyer), Functionality of the Service to which the complaint pertains (in the case of a complaint submitted by the Buyer), the subject of the complaint with an indication of the Buyer's request and all circumstances justifying the complaint.
- If the data or information provided in the complaint needs to be supplemented for the complaint to be considered properly, the Seller shall request the Complainant, via the address or e-mail address provided, to supplement the complaint to the extent and by the date indicated, before considering the complaint.
- The Seller shall assess the subject of the complaint and its legitimacy within 30 (thirty) days from the date of its 1TP2Note. The Seller shall inform the Complainant of the refusal of the complaint and the reasons for it in writing or by sending a message to the Complainant's e-mail address - depending on the form of the complaint. If the Seller does not refer to the complaint within 30 (thirty) days from the date of its receipt, the complaint shall be deemed justified.
- The seller shall refund the payment for the complaint by traditional bank transfer to the designated account provided by the buyer, unless the buyer has expressly indicated a different method of payment that does not incur any costs for him.
- If a dispute arises between the Seller and the Complainant as to the legitimacy of the refusal to accept the complaint, the Complainant has the right to use out-of-court ways of handling complaints and pursuing claims. The Complainant may use mediation or an amicable court by delivering to the institution before which the proceedings will take place, an appropriate form - application for mediation or application for consideration of a case before an amicable court. A list of sample institutions together with their contact details is available on the gov.pl website.
- The Buyer has the right to withdraw from the Sales Agreement of the Products without giving any reason by sending a statement of withdrawal to the Seller at the e-mail address: recepcja@odent.pl or in writing to the Administrator's registered office address.
- The deadline for withdrawal from the Product Sales Contract is 14 days from the date of conclusion of the Contract.
- The Buyer shall have the right to withdraw from the Service Contract only if he has not commenced using the Services and no later than 14 days from the date of the conclusion of the Service Contract, i.e. making the Upfront Payment. In such a case, the Seller shall immediately, but not later than within 14 days from the date of receipt of the notice of withdrawal from the Service Contract złaken by the Buyer, refund to the Eligible Person all payments made by him for the Service.
- The Seller shall immediately confirm to the Eligible Person at the e-mail address provided the receipt of the notice of withdrawal from the Service Contract or the Product Sales Contract.
- In the event of withdrawal from the Service Contract or the Product Sales Contract, the Service Contract or the Product Sales Contract, respectively, shall be deemed not to have been concluded.
- The Seller shall immediately, but not later than within 14 days from the date of receipt of the declaration of withdrawal from the Service Contract or Product Sales Contract 1TP2Forged by the Buyer, return to the Eligible Person all payments made by him for the Service or Product.
- The seller shall refund the payment by traditional bank transfer to the nominated account provided by the buyer, unless the buyer has expressly indicated a different payment method which does not incur any costs for him.
- The Seller shall only allow the Buyer to withdraw from the services provided if there are medical contraindications to the continued performance of the Treatments, in particular if, after the conclusion of the Contract for Services, any of the contraindications to the performance of the Services contained in the consent for the relevant Treatment arise.
- On a case-by-case basis, the seller may allow the buyer to cancel some of the services.
- In order to resign from a part of the provided services, the Buyer should notify the Seller of his/her wish to resign from further use of the services by sending a statement of resignation to the Seller at the e-mail address: recepcja@odent.pl or in writing to the Administrator's registered office address - within 14 days from the discovery of any health contraindications, under pain of losing this right.
- The notice of cancellation of the Services should include at least: the name, surname, address, electronic mail (e-mail) address of the Buyer, an indication of the basis for the cancellation (medical contraindication to further use of the Treatments), the date on which the contraindication to further use of the Treatments was discovered and any circumstances justifying the cancellation.
- If the data or information provided in the cancellation needs to be supplemented for the cancellation to be processed correctly, the seller will ask the buyer to supplement the data or information to the specified extent and by the specified deadline via the given address or e-mail address before processing the cancellation.
- The evaluation of the subject of the cancellation and its validity shall be carried out by the Seller within 14 (fourteen) days from the date of its złożenia.
- The Seller shall inform the Buyer in writing or by sending a message to the Buyer's e-mail address - depending on the form in which the cancellation is notified - of the refusal to accept the cancellation and the reasons for it.
- The price will be refunded within 14 (fourteen) days of the successful cancellation.
- The seller will immediately confirm receipt of the cancellation notice to the buyer at the e-mail address provided by the buyer.
- The seller shall refund the payment by traditional bank transfer to the nominated account provided by the buyer, unless the buyer has expressly indicated a different payment method which does not incur any costs for him.
- The prices given in the online shop include VAT and are expressed in Polish currency. The prices do not include shipping costs, the Buyer is informed about the shipping amount after each złransfer of the order.
- Changes to the terms and conditions that arise from the necessity of adapting them to the current state of the law, registered buyers will be notified each time by e-mail. The Seller undertakes to send information about the scope of the changes at least 30 days before the new terms and conditions come into force.
Information on the scope of changes will be sent to the e-mail address indicated by the Buyer during registration. Under the “Rules and Regulations” tab on Odent.pl's website, you will always find the current version of the applicable rules and regulations. The Buyer is bound by the version of the Rules and Regulations that was accepted when placing the order in the online shop.
The Seller undertakes to protect personal data in accordance with the Personal Data Protection Act of 29.8.1997 (Journal of Laws No. 133, item 883) and the Act on Electronic Service Provision (Journal of Laws No. 144, item 1204). By providing his or her personal data when placing an order, the buyer agrees to the processing of that data for the purpose of carrying out the order. The buyer has the right to access, modify and delete their personal data.

The Seller shall ensure that the Buyer's online shop works correctly in the following browsers: Chrome version 10 or later, Firefox version 3 or later, Safari with up-to-date versions of JAVA and Flash installed, on screens with a horizontal resolution above 1024 px.
(10) In matters not regulated herein, Polish law shall apply, including the relevant provisions of the Civil Code and the Code of Civil Procedure as well as the Act on the Protection of Certain Consumer Rights.
The administrator of the data is Centrum Medyczne ODENT Sp. z o.o. with its registered office in Warsaw, 85/U4 Grzybowska Street. The personal data provided will be processed in order to:

- Telephone contact to remind you of your appointment.
- Email contact with the newsletter sent.
- Text message contact with newsletter sent.
The recipients of the data we process are:
- authorised employees of the Data Controller.
In connection with the processing of your personal data, we do not pass it on to third parties.
Personal data provided on a compulsory basis:
- Patient's name,
- Email address,
- Phone.
In relation to the processing of your personal data, you have the right to:
- Access to data;
- Data corrections;
- Deletion of data - to the extent provided for by applicable legislation;
- Restrictions on processing - to the extent provided for by applicable legislation;
- To object to the processing;
- Data portability - to the extent provided for by applicable legislation;
- Revoke your consent to process your data on the basis of your prior consent without affecting the lawfulness of the processing carried out on the basis of your consent before revocation;
- Lodge a complaint with a supervisory authority - if you consider that the processing of your data is not in compliance with the applicable legislation.
