Terms and conditions for the implementation of the Voucher - Centrum Medyczne ODENT Sp. z o.o.
§ 1
- These Terms and Conditions set out the rights and obligations of the holder of the Voucher and the rules for receiving and redeeming it („Voucher”), pursuant to Article 384 of the Civil Code.
The voucher is a document issued by Centrum Medyczne ODENT sp. z o.o. with its registered office in Warsaw, ul. Grzybowska 85/U4, 00-844 Warsaw, registered in the National Court Register, NIP: 5252887815, REGON: 520631198 (hereinafter: ODENT). - The Voucher may be redeemed in accordance with these Terms and Conditions and applicable laws.
§ 2
- A voucher is a document that allows you to provide selected medical services, receive certain products, receive a discount on services or products provided by ODENT or make a payment for a service provided by ODENT.
- The voucher is issued in paper form, containing the type of service, a specific product, the amount of discount or amount to be used and - optionally - a description of how to redeem it.
- The voucher is valid for 12 months from the date of issue.
- The Voucher is not exchangeable for cash. Any unused excess value of the Voucher will not be refunded.
- If the value of the services rendered exceeds the value of the Voucher, the Customer is obliged to pay the difference in cash, by card or bank transfer, according to the price list in force on the date of redemption.
- A voucher issued for a specific service cannot be used for another service.
- Discounts, promotions, other discounts and vouchers do not add up.
- The quota voucher cannot be used as a form of payment for services covered by a promotion or discount.
- A purchased Voucher is non-refundable. Vouchers not used before the expiry date are not accepted for return.
§ 3
- The prerequisite for using the Voucher is to call ODENT in advance, arrange an appointment and provide the Voucher number and expiry date.
- The condition for redemption is that the Voucher is handed over in the original before the service is provided or the product is purchased.
- ODENT reserves the right to exclude certain dates from the implementation of the Voucher for legitimate organisational reasons.
- The Reception is entitled to verify the validity of the Voucher and, in the case of a named Voucher, the identity of the person redeeming it. In the event of a discrepancy, ODENT may refuse to redeem and retain the Voucher.
- The holder of the Voucher may change the date of the visit no later than 2 working days before the planned visit otherwise the Voucher will be forfeited.
- The Voucher holder is responsible for its loss; duplicates are not issued.
§ 4
- ODENT may refuse to redeem the Voucher (provide a service, issue a product, give a discount or credit the value of the Voucher) for legitimate reasons.
- The Voucher shall only be redeemed after it has been handed over to an ODENT employee.
§ 5
Each Voucher holder declares that they understand and accept that:
- The voucher is the property of ODENT.
- It is prohibited to resell, commercialise, dispose of, lend, distribute or transfer the rights to the Voucher to third parties.
- The Voucher may only be redeemed by the person indicated on the Voucher or by an authorised person based on a separate decision of ODENT.
- It is forbidden to modify the content of the Voucher.
- Infringement of the rules for the use of the Voucher may result in liability under the industrial property laws.
- The Voucher Holder undertakes to use the Voucher in accordance with the Terms and Conditions.
- ODENT shall be liable for the non-performance or improper performance of the obligations set out in these Regulations.
§ 6
- Complaints regarding the implementation of the Voucher can be made in writing or electronically within 14 working days from the date of use of the service.
- Complaints are dealt with within 14 working days of receipt.
- The customer will be informed of ODENT's decision in writing.
§ 7 - Personal data protection
- The administrator of the personal data is Centrum Medyczne ODENT sp. z o.o.
- The Customer's personal data shall only be processed for the purposes of providing the services and for other purposes to which the Customer has consented.
- The customer may consent to the processing of data for marketing purposes and to receive commercial information.
- ODENT uses technical and organisational measures in accordance with the RODO.
- The customer agrees to the processing of personal data to the extent necessary for the provision of services.
§ 8 - Information on personal data
- The administrator of the Customer's personal data is Centrum Medyczne ODENT sp. z o.o., ul. Grzybowska 85/U4, 00-844 Warsaw.
- Personal data is processed for the purpose of providing medical services.
- Personal data may only be shared with other parties on the basis of legal provisions.
- Data will not be transferred to third countries or international organisations.
- Data will be stored in accordance with the legislation on medical records and medical services.
- The customer has the right to access, rectify, erase, restrict processing, portability and object to their data.
- The customer has the right to lodge a complaint with the President of UODO.
- The provision of data is necessary for the performance of the service; failure to do so prevents the provision of the service.
- Data may be processed by automated means, including profiling - only with consent.
- Detailed information can be found in the ODENT Privacy Policy available at the facility and on the website.
§ 9
- ODENT is not responsible for refusing to perform a procedure if medical contraindications are found.
- ODENT is not responsible for damage to or loss of the Voucher.
- Failure to use the Voucher by the deadline means that it is no longer valid and does not give rise to a claim.
- The Voucher Holder declares that he/she has read and accepts the Terms and Conditions.
- The regulations are available at the ODENT branch and on the ODENT website.
- ODENT reserves the right to amend the Terms and Conditions, whereby the amendments do not affect the acquired rights of customers.
- In matters not regulated, the Regulations for the Provision of ODENT Medical Services shall apply.
- In matters not regulated, the provisions of the Civil Code shall apply.
